If your student ID card has been lost or stolen, please call the number below immediately.

Hours of office: Everyday 09:00－18:00
If not found, please fill in a Request form ( Faculty of Engineering / School of Engineering ).
If you’re requesting a new student ID card because your old one was lost, stolen or damaged, you are required to pay a re-issuing fee of 2000 JPY.
Please submit the form with the payment receipt or payment statement attached to the appropriate space of the request to the section listed above.
It will take about two weeks to re-issue your card.
・Faculty of Engineering:
Please bring the form to Undergraduate Team Office (1st floor, Engineering Building 8, weekdays from 9:00 to 14:00).
・School of Engineering:
Please bring the form to Graduate School Team Office (1st floor, Engineering Building 8, weekdays from 9:00 to 14:00) or send it by email to Graduate School Team at (daigakuin.t@gs.mail.u-tokyo.ac.jp).
